Directions for completing Work Plan:
This evaluation is to be completed at the end of the spring semester. Field hours should be completed each semester before the last day of final exam period but no sooner than the last week of classes. Subsequently students and field supervisors should meet to discuss the evaluation prior to submission. The field supervisor will, using the form below, rate students on each of the behaviors. Based on the feedback from the student, field supervisor, the faculty field liaison will assign a grade for the student.
